Experience in heart transplantation after dynamic cardiomyoplasty.

چکیده
We report a case undergoing heart transplantation due to gradual progression of heart failure four years and one month after dynamic cardiomyoplasty. This case reports a 61-year-old man who received drug therapy after being diagnosed as having idiopathic dilated cardiomyopathy, but his heart failure progressed to New York Heart Association (NYHA) class III-IV, and heart transplantation was thought to be indicated. However, dynamic cardiomyoplasty was performed because this patient rejected heart transplantation. An implantable cardioverter/defibrillator (ICD) was implanted for postoperative ventricular arrhythmia. After that, his symptoms rapidly improved, but his heart failure gradually worsened two years after surgery and heart transplantation was performed four years and one month after dynamic cardiomyoplasty. Since transplantation he has had an uneventful postoperative course without rejection or complications.

Limited systolic assistance by weak power of the Latissimus Dorsi Muscle (LDM) due to excessive slow speed of shortening have been considered a problem of Dynamic CardioMyoPlasty (DyCMP ) after its creation by Carpentier and Chachques. Dynamic Cardiomyoplasty (DCMP) aims at improving cardiac function in cases of severe heart failure. It consists in anchoring Latissimus Dorsi Muscle (LDM) to failing ventricles. LDM is stimulated in synchrony with ventricular contractions by burst of impulses (6, 31 ms) delivered by a cardiomyostimulator (CM). A retrospective analysis of collective French experience (6 centers) is reported. Sin...
متن کاملDynamic cardiomyoplasty for heart failure.
In dynamic cardiomyoplasty the latissimus dorsi of the patient is lifted as a pedicle graft, placed in the chest cavity, and the muscle is wrapped around the ventricles of the failing heart.' This muscle undergoes low frequency electrical stimulation for several weeks to confer fatigue resistance. The muscle is then stimulated by a synchronisable burst stimulator to contract during cardiac syst...

Tissue or Cell Cardiac Bioassistance (Cardiac-Bio-Assists) is a biological approach to remedy to cardiac progressive insufficiency based on autologous tissue or cell transplantation.
